Before you send in your favorite photo for processing please read the attached information that explains what to look for when choosing a photo as well as answers to many of the most frequently asked questions.

Send your photo and this completed form along with your payment to the address below. Please allow four to six weeks from the date you put your order in the mail for your chart, easy instructions and your original photo to return to you. For faster service see back of form for other processing/mailing options.

IMPORTANT REMINDERS: Busy “B” Crafts cannot be held responsible for photographs lost in the mail. Make sure that you have an extra copy of the photo you are sending along with this order form.
